The incident investigation is a reverse engineering practice which identifies gaps/ failures/ lapses in the processes and propose preventing measures towards avoiding reassurance.

In this new webinar, a practical approach on STS incident investigation will be presented in order to emphasize on major identified errors in practices that usually lead to incidents.

Inadequate Incident Management is a possible causal factor in OCIMF STS guidelines and as such, findings from past incidents should be systematically communicated within the organization.

A.Martyn Haines, Associate Legal Contractor of DYNAMARINe

Martyn is a Master mariner and has served in many different ship types, from oil tankers to heavy lift cargo carriers. He has worked in post-fixture commercial operations and was the fleet manager of over 30 refrigerated cargo ships, before moving into P&I insurance in 1996.Martyn rose to the position of Senior Claims Director before being headhunted by a leading London Law Firm, to be a casualty investigator in 2011. In 2019, Martyn became an independent marine consultant and commercial mediator after accreditation with Civil Mediation Council. Martyn has worked with DYNAMARINe since 2012.

B. Dr.Kirikos Faraklas, LRQA Marine Manager, Business Assurance @Lloyd's Register

Dr. Kirikos Faraklas is the LRQA Marine Manager, Business Assurance of Lloyd's Register. He is a Management System Lead Tutor, Lead Assessor and Lead Verifier. He joined Lloyd's Register in 2005. From 1991 to 2002 he worked in the petrochemical industry and since 2002 he is working in the safety, quality and environmental management systems sector. He has a Chemical Engineering Degree from the National Technical University of Athens, a Doctor of Philosophy (PhD) and a Diploma of Imperial College (DIC) from the University of London.
